Certificate of Death
The physician who certified the death, or the Funeral Director files the Death Certificate with the Registrar of the Municipality where the death occurred.
Certified Copy of Death
Certified copies must be acquired where the incident occurred regardless of your place of residence. The New Jersey State Division of Vital Statistics also has all certificates on file. The fee for a certified copy of death is $15.00 for each copy. No charge shall be made when copies of documents are required by a federal agency for use in determining the eligibility of a person to participate in benefits made available by the federal agency.
