OPEN ENROLLMENT

October is the SHBP Open Enrollment Period - your annual opportunity to review health benefits coverage and make any changes for you and/or your dependents

During the Open Enrollment you may:

- Enroll in SHBP plans if you have not previously done so;
- Change to a different SHBP medical plan
- Add eligible dependent - in children up to age 26* under federal health care reform changes

To change or enroll in SHBP plans, contact your human resources representative or benefits administrator to obtain an application. Medical and prescription drug coverage changes are made of the same application.

Changes made during Open Enrollment will be effective January 1st for all employees

*Optional coverage for dependent children until age 31 is available under the provisions of Chapter 375, P.L. 2005

Changes to Health Benefits

As a City Employee, it is your responsibility to notify the Payroll/Personnel Office of any life-changing events such as change of address, births, marriages, divorces, or deaths. Below, you will find all forms necessary to notify the Payroll/Personnel Office. All forms are set up so that you can fill in all of the information required on your computer to print and submit.

MBOS Information

The New Jersey Division of Pensions and Benefits is now online. If you have any questions regarding your pension, including pension loans, you are able to check into this information on your own by registering with the Member Benefits Online System (MBOS). Below is step-by-step instructions as to how to find all of this information online including creating a log in.

Council 18 Return to Work Form

Pursuant to an agreement between the City of Millville and Civil Service Association Cumberland Council No. 18, any employee who is absent from their work duties for five days or more, but less than ten days must submit the below Return to Work Form completed by the employee's treating doctor, which states clearly that the employee can return to work "with no employment 'restrictions' or 'conditions".
