Grade 6 Immunization Requirements

Students 11 years of age and entering grade 6 are required to receive 2 additional immunizations.  These are:

Tdap - Tetanus, Diptheria and Pertussis

Meningococcal vaccine

When your child turns 11, please contact his/her healthcare provider to schedule the administration of these vaccines.  Bring documentation of receipt of these vaccines to the school nurse.

If your child will turn 11 during the summer between grades 5 and 6, bring proof of immunization to the nurses at Lakeside Middle School (all student health records for students entering grade 6 are transferred to Lakeside Middle school at the end of the school year).

If your child will turn 11 on or after the first day of grade 6, he/she will have 2 weeks to receive these vaccines.

Students must receive these immunization or have a valid religious/medical exemption in order to attend grade 6.
