Important Information for parents of students turning 11 and entering grade 6:

The State of New Jersey has added 2 vaccine requirements for students entering grade 6.  Both are given after the age of 11.  These requirements are:

  • Tetanus-diptheria-pertussis vaccine(Tdap)
  • Meningococcal vaccine

It’s also a great time to discuss the HPV (Human papilloma virus vaccine) with your child’s healthcare provider. 

If your child will be 11 years old before the first day of 6th grade, please do the following:   

  • Make an appointment with your child’s healthcare provider to get these vaccines.
  • Bring the school nurse documentation from your healthcare provider indicating that these shots were given
  • If your child turns 11 during the summer, take this documentation to the middle school nurses.

If your child turns 11 after the first day of school, he/she will have 2 weeks to provide proof of immunization.
