• 2017-2018 SPORTS PHYSICAL PACKETS

    The new 2017-2018 Sports Physical Packet is now available. Effective July 1, 2015 all new physicals MUST be completed on the new packet by the athlete's doctor*.

    *PLEASE NOTE THE FOLLOWING*
    The Pre-Participation Physical Evaluation may ONLY be completed by a licensed physician,
    advanced practice nurse (APN) or physician assistant (PA) that has completed the
    Student-Athlete Cardiac Assessment professional development module. 
    It is recommended that you verify that your medical provider has completed
    this module before scheduling an appointment for a PPE.
     
    Packets are available in the Main Office at Memorial, Lakeside, and the Senior High. You can also print a copy of the packet by clicking on the link below. If you have any questions, please call the Athletic Office at 856-327-6059.
     
     

    2017 SUMMER AND FALL SPORTS SEASONS

    Summer Weight Training and Summer Practice Sessions will begin Monday, June 26, 2017. The Fall Sports Season will begin on Wednesday, August 9, 2017. Any students who want to participate must have a current sports physical on file in the nurse’s office. Sports physicals are valid for 365 days from the date of exam. If your physical date is more than 90 days old, you must complete a recert form. All forms must be turned in to the school nurse. Sports Physical Forms may take up to 10 days to be cleared and Recert Forms may take up to 48 hours to be cleared. Only athletes who are cleared can participate – no exceptions.     SCHOOL SPONSORED SPORTS PHYSICALS (Effective June, 2014)

    We would like to inform you that Millville Public School District will no longer be sponsoring free sports physicals during the summer with our school doctor. Students will need to have their sports physical completed by their own physician. Students who do not have a physician can contact their respective school nurse to discuss. There are no exceptions to this rule. Any students who wish to participate in any sports throughout the school year must have a current sports physical on file in the nurse's office.
