Immunization Requirements

Immunization Requirements

The New Jersey Department of Health requires that all children attending school receive required immunizations. Failure to ensure your child receives these immunizations will prevent them from attending.

Preschool
  • Tetanus, Diptheria, and acellular Pertussis (Tdap) - 4 Dates
  • Inactivated Poliovirus (Polio) - 3 Dates
  • Hoemophilus influenoe Type B (HiB) - 1 Date after 1st Birthday
  • Pneumococcal conjugate (PCV 13) - 1 Date after 1st Birthday
  • Measles, mumps, rubella (MMR) - 1 Date after 1st Birthday
  • Varicella (VAR/Varivax) - 1 Date after 1st Birthday / written proof of Chicken Pox disease
  • Influenza (IIV; LAIV) - Between 9/1 and 12/31 each year

Kindergarten
  • BOOSTER: Tetanus, Diptheria, and acellular Pertussis (Tdap)

  • Hepatitis B (3 doses)
