Household Contact Changes

 
Parents/Guardians must make an appointment with the Registration Office to make any changes to household information OR send the required documentation by email.
 
These changes cannot be made with your child's school.

You must provide the following documents for household contact changes:
 
If sending request via email, please include scans of photo ID and documents as attachments.
 
  • Parent/guardian for household
    • Photo ID of parent/guardian requesting change
    • Marriage certificate
    • Adoption paperwork
    • Custody paperwork
    • Any other proof of legal guardianship
 
  • Household phone number
    • Photo ID of parent/guardian requesting change
    • Previous household phone number
    • New household phone number
 
    • Cell phone/Work phone/Other phone numbers for individual contacts may be made in the Parent Portal.
    • Email address changes for individual contacts may be made in the Parent Portal.
 
  • Household/non-household/emergency contacts
    • Photo ID of parent/guardian requesting change
    • Name and contact information of the new contact
    • Name and contact information for any contact you wish to remove
