Household Contact Changes

 
Parents/Guardians must visit the Registration Office to make any changes to household information OR send the required documentation by email.
 
  • Household phone & Cell phone/Work phone/Other phone numbers for individual contacts may be made in the Parent Portal.
  • Email address changes for individual contacts may be made in the Parent Portal.

Please bring a photo ID with you if visiting in-person.
 
 
You must provide the following documents for household, non-household, and emergency contact changes:
 
If sending request via email, please include scans of photo ID and documents as attachments.
 
  • Parent/guardian for household
    • Photo ID of parent/guardian requesting change
    • Marriage certificate
    • Adoption paperwork
    • Custody paperwork
    • Any other proof of legal guardianship
 
  • Household/non-household/emergency contacts
    • Photo ID of parent/guardian requesting change
    • Name and contact information of the new contact
    • Name and contact information for any contact you wish to remove
