Household Contact Changes
Parents/Guardians must visit the Registration Office to make any changes to household information OR send the required documentation by email.
- Household phone & Cell phone/Work phone/Other phone numbers for individual contacts may be made in the Parent Portal.
- Email address changes for individual contacts may be made in the Parent Portal.

Please bring a photo ID with you if visiting in-person.
You must provide the following documents for household, non-household, and emergency contact changes:
If sending request via email, please include scans of photo ID and documents as attachments.
- Parent/guardian for household
- Photo ID of parent/guardian requesting change
- Marriage certificate
- Adoption paperwork
- Custody paperwork
- Any other proof of legal guardianship
- Household/non-household/emergency contacts
- Photo ID of parent/guardian requesting change
- Name and contact information of the new contact
- Name and contact information for any contact you wish to remove
